To request an on-the-day telephone appointment, use the form below. A clinician will contact you in the morning (AM) or afternoon (PM), depending on your choice. Please keep your phone nearby and be ready to answer, as the call may come from a withheld number.

When completing the form, provide as much detail as possible about the reason for your appointment. This will help the clinician prepare before calling you.

This service is for telephone consultations only. If the care advisor decides that a face-to-face appointment is more appropriate, they will contact you to arrange this. If you require a face-to-face or another type of appointment directly, please contact the practice in the usual way.

The form is available daily between 5:00am and 8:00am. If there are no appointments available, please try again the next working day or contact the practice as normal.

Please note: This form must only be completed for the patient named on the form. If you are requesting an appointment for someone else, or for more than one person, a separate form must be submitted for each individual.
